But nobody knows the future, and sometimes, you have to make the best decision you can based on the information you have. If the outcome is bad because of unexpected, unplanned, unanticipated events - does that mean you made a bad decision at the time?

"Never judge a decision by its outcome" could be a very sharp double-edged sword.
For employees: They don't get fired just because there were some bad results while they were honestly doing heir best.
For top management: They don't get blamed even if they sent their whole company down in flames…???
