75% of executive escalations I get can be summed up as an off hand comment picked up by staff and blown up to be a major issue. When addressed to the executive, they usually look baffled about what anyone was taking action.

Yup. Empowered by the mid-tier freaks who are terrified of anything disappointing the boss.

You can sub "flag officer" for "ceo" and it's the exact same way in the service.
